Position Details

  • Schedule: 5 days per week, 8-hour days (no weekends or on-call requirements)
  • Weekly pay: $2,534.40

Qualifications Required

  • Minimum 1 year of physical therapy experience
  • New graduates accepted with Level II clinical experience (Level I observation alone is insufficient)
  • Long-term care or rehabilitation experience required
  • Valid Oklahoma state physical therapy license

Key Responsibilities

  • Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
  • Develop and implement plans of care following regulatory and clinical standards
  • Utilize standardized assessments and evidence-based practice for clinical interventions
  • Administer procedures including manual techniques, ambulation training, therapeutic exercises, modalities, assistive devices, and wound care
  • Monitor patient progress and adjust treatments in the clinical record
  • Evaluate treatment outcomes and modify services as needed
  • Provide consultation and counseling to patients, families, caregivers, and service providers
  • Communicate with supervisors and health team members regarding patient progress and treatment plans
  • Prepare comprehensive discharge summaries per regulatory requirements
  • Document all required information in the electronic system and maintain accurate patient records
  • Record treatment minutes for services provided per regulatory standards
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Complete facility and company required trainings and in-services
  • Obtain necessary DME and supplies per regulatory and clinical practice requirements
  • Supervise and support physical therapist assistants, rehabilitation aides, and students
  • Adhere to APTA and state-required practice acts, code of conduct, and ethics standards
