PT - Physical Therapist

Job Details:

• Home health skilled visits with 6-8 visits per day (minimum of 6 points per day, may increase at manager's discretion)
• Every 3rd weekend required
• Self-scheduled visits and hours based on the needs of the HomeCare office
• Serve patient population including cardiopulmonary, geriatrics, neuro, ortho, and oncology specialties
• Must have a valid driver's license and personal vehicle to conduct home visits
• 2 years of experience required
• Home health experience required; OASIS training preferred
• Required certifications: BLS and active state license
• Required skills include assessment, cardiopulmonary/circulatory care, documentation, ortho, neuro, home exercise program development, medications, patient and family education, safety and injury prevention, and signs and symptoms of physical, sexual, and psychological abuse
• Must be proficient with equipment and devices including crutches, gait belt, hospital bed, rolling walker, shoulder slings, sliding boards, soft/hard collars, standard walker, and supplemental oxygen delivery
• Laptop, MiFi for internet access, and portable printers provided
• Documentation completed using BayCare Forms, BayCare Scheduling, and Ace Timesheet system
• Scrubs: Any solid color scrub
