Physical Therapist Home Health Job in Lakeland, FL

ADEX Healthcare Staffing is seeking a Physical Therapist for home health with BayCare HomeCare Inc. in Lakeland, Florida. This is a 140-day travel assignment running 5 days per week, 8-hour shifts, with a weekly pay of $2,490. Start date is November 16, 2026.

Role Overview

You will conduct skilled home visits with a minimum of 6–8 patients per day (minimum 6 points daily, subject to increase at manager discretion). You'll work with adult and geriatric populations across multiple clinical areas: medical/surgical, cardiac, orthopedic, post-surgical recovery, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's disease, respiratory conditions, neurological disorders, trauma, amputation (AKA/BKA), and activities of daily living (ADL) support.

Clinical Responsibilities

• Perform patient assessments, evaluations, and reassessments for Medicare, Medicare Advantage, and commercial insurance coverage
• Develop and implement home exercise programs, plan of care, and short- and long-term rehabilitation goals
• Provide gait training, patient and family education on safety, injury prevention, and abuse recognition
• Utilize adaptive and assistive devices: gait belts, rolling walkers, sit-to-stand lifts, sliding boards, cervical collars, supplemental oxygen, upper extremity prosthetics, and wheelchairs
• Document assessments and interventions using facility-specific forms and the ACE Timesheet documentation system

Work Environment

You will use a provided laptop, MiFi internet access, and portable printer for documentation. Block scheduling applies. One to two weekend shifts per contract are required. A valid driver's license and personal vehicle are mandatory for home visits.

Required Qualifications

• Active Florida or Compact state physical therapy license
• Minimum 1 year of physical therapy experience
• Home health experience (OASIS training preferred)
• Current BLS certification
• Core competencies: OASIS, orthopedics, neurology, cardiopulmonary conditions, gait training, patient and family education, billing, and documentation
