Physical Therapist Home Health Job in Tampa, FL

BayCare HomeCare Inc. is seeking a Physical Therapist for home health services in Tampa, Hillsborough County, Florida. This is a 140-day travel assignment running from November 16, 2026, with a 5x8 day shift schedule and block scheduling that includes 1–2 weekend shifts per contract.

Role and Responsibilities

• Perform skilled home care visits for patients covered under Medicare, Medicare Advantage, and Commercial insurance
• Complete 6–8 home visits per day, with a minimum of 6 points per day (may increase by 1 point at manager discretion)
• Treat adult and geriatric patients across a wide range of diagnoses including medical/surgical, cardiac, orthopedic, hip/knee/shoulder, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's disease, failure to thrive, respiratory, neurological, tracheostomy, trauma, AKA, BKA, and ADLs
• Utilize provided equipment: laptop, MiFi for internet access, and portable printers
• Document using BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system

Requirements

• Valid state Physical Therapist license
• BLS certification
• Valid driver's license and personal vehicle for home visits
• Minimum 1 year of physical therapy experience
• Home health experience required; OASIS training preferred
• Required clinical skills: assessment, cardiopulmonary/circulatory evaluation, adaptive and assistive device use (gait belt, rolling walker, sit-to-stand lift, sliding boards, cervical collars, standard walker, supplemental oxygen, upper extremity prosthetics, wheelchair assessment), orthopedic and neurological assessment, home exercise program design, medication knowledge, patient and family education, safety and injury prevention, and billing
• Dress code: solid color scrubs

Compensation and Duration

Weekly pay: $2,490. Assignment length: 140 days.
