PT - Physical Therapist

Job Details:

• Home health skilled visits with 6-8 patient visits per day (minimum of 6 points per day, may increase by 1 point at manager's discretion)
• Every 3rd weekend required
• Visits and hours are self-scheduled based on the needs of the HomeCare office
• Serve patients across cardiopulmonary, geriatrics, neuro, ortho, and oncology specialty populations
• Adult and geriatric patient age groups
• Must have a valid driver's license and personal vehicle to conduct home visits
• 2 years of experience required
• Must have home health experience; OASIS training preferred
• Required certifications: BLS and active state license
• Scrubs: Any solid color scrub
• Required skills include assessment, cardiopulmonary/circulatory care, documentation, ortho, neuro, home exercise programs, medications, patient and family education, safety and injury prevention, and signs and symptoms of physical, sexual, and psychological abuse
• Equipment experience required: crutches, gait belt, hospital bed, rolling walker, shoulder slings, sliding boards, soft/hard collars, standard walker, and supplemental oxygen delivery
• Laptop, MiFi for internet access, and portable printers provided
• Documentation systems used: BayCare Forms, BayCare Scheduling, and Ace Timesheet
