PT - Physical Therapist Acute Care

Job Details:

• Work 5x8 day shifts, Tuesday through Friday 10:00 AM to 6:30 PM and Saturday 8:00 AM to 4:30 PM
• Plan, prepare, and implement individually designed physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients
• Complete and document patient evaluations and tests per established standards and regulatory requirements
• Develop and update patient Plans of Care (POC) based on exam data, identified problems, and diagnosis
• Assess and provide treatment for patients per plan of care and established standards
• Complete all documentation related to charge capture per established standards
• Coordinate patient care with other care providers and support team members
• Provide clinical supervision to assistants and/or aides
• Serve as clinical instructor for assigned students
• Participate in quality improvement processes and interdisciplinary projects
• Inspect and use equipment per manufacturer guidelines, recording data and reporting malfunctions per policy
• See patients across adult and geriatric age groups with an average of 7 visits per day
• Epic charting system experience required
• Community hospital experience preferred
• Graduate of an accredited PT program required
• Active PT licensure required (approved license applicants or resident training program employees will also be considered)
• BLS (AHA) certification required
• Minimum 2 years of experience required
