Occupational Therapist Job in Ada, OK

Tide Health Group - Ada Care Center is seeking an Occupational Therapist for a temporary position in our long-term care facility. You will work a 5x8 day shift schedule with no on-call or weekend responsibilities, providing direct patient care and clinical leadership in a skilled rehabilitation environment.

Your Responsibilities:

• Screen and evaluate patients with cognitive and physical functioning disorders; develop individualized plans of care
• Administer standardized assessments and evidence-based interventions, including ADL retraining, therapeutic activities, therapeutic exercises, and modalities
• Assess patient progress and adjust treatments accordingly; evaluate outcomes for effectiveness and modify services as needed
• Provide rehabilitation and medically necessary treatment interventions in accordance with regulatory and clinical practice requirements
• Consult with patients, families, caregivers, and service providers regarding treatment plans and physical disorders
• Communicate with supervisors and healthcare team members regarding patient progress, barriers, and care adjustments
• Document comprehensively in the electronic health record system, including treatment minutes, clinical notes, and discharge summaries per regulatory standards
• Participate in patient care conferences, utilization review meetings, and rehabilitation team meetings
• Supervise and support occupational therapist assistants, rehabilitation aides, and students

Required Qualifications:

• Bachelor's or Master's degree in Occupational Therapy
• Active Oklahoma OT license at time of submission
• Minimum 1+ years of occupational therapy experience
• Long-term care or rehabilitation experience required
• NBCOT certification and compliance with state practice acts and professional code of conduct
• Skills checklist completion within 1 year of start date
