Occupational Therapist Job in Ada, OK

Tide Health Group - Ada Care Center is seeking an Occupational Therapist for a long-term care setting in Ada, Oklahoma. This is a temporary position running 91 days, starting July 27, 2026.

Position Details

• Schedule: 5x8 days per week
• Weekly pay: $2,641
• No on-call responsibilities
• No weekend requirements

Your Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Provide rehabilitative, skilled, and medically necessary treatment interventions per regulatory and clinical practice requirements
• Administer rehabilitation procedures including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments accordingly in the clinical record
• Evaluate treatment outcomes for effectiveness and modify services in relation to patient evaluation
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records according to company policy
• Accurately record treatment minutes for services provided per all regulatory and clinical requirements
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support occupational therapist assistants, rehabilitation aides, and/or students

Requirements

• Active Oklahoma OT license required at time of submission
• Minimum 1+ years of OT experience
• Long-term care or rehabilitation experience required
• Skills checklist completed within 1 year required
• Adherence to NBCOT and state required practice acts, code of conduct, and ethics during professional practice
