Job Details:

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Provide rehabilitative, skilled, and medically necessary treatment interventions per regulatory and clinical practice requirements
• Administer various rehabilitation procedures including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments accordingly in the clinical record
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
• Communicate effectively with supervisor and health team members regarding patient progress, barriers, and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records according to policy
• Accurately record treatment minutes for services provided per all regulatory and clinical requirements
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support occupational therapist assistants, rehabilitation aides, and/or students
• Adhere to NBCOT and state required practice acts, code of conduct, and ethics during professional practice
• No on-call responsibilities
• No weekend requirements
• 1+ years of OT experience required
• LTC or Rehab experience required
• Active OT license required at time of submission
• Skills checklist completed within 1 year required
• 0-1 reference required at time of submission
