SLP Job in Hondo, TX

Tide Health Group – Community Care Center of Hondo is seeking a Speech-Language Pathologist (SLP) for a long-term care position. You will work days, 5 days per week, 8 hours per shift, with no on-call or weekend requirements. This is a 91-day assignment beginning September 21, 2026, at $2,448 weekly.

Your Responsibilities

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
• Develop individualized plans of care aligned with regulatory and clinical practice standards
• Use standardized assessments and evidence-based interventions to support patient outcomes
• Monitor patient progress and modify treatment approaches based on clinical response
• Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
• Maintain clear communication with supervisors and staff regarding patient progress, barriers, and treatment modifications
• Document all services in the electronic health record according to company and regulatory requirements
• Accurately record treatment minutes per regulatory guidelines
• Participate in patient care conferences, utilization review meetings, and rehabilitation planning sessions
• Complete required facility and company trainings and in-services
• Support or supervise Clinical Fellow Year (CFY) or student Speech-Language Pathologists as needed
• Obtain necessary durable medical equipment and supplies per clinical standards

Requirements

• Texas State License required
• Certificate of Clinical Competence (CCC) from ASHA required
• Minimum 1 year SLP experience required
• Long-term care or rehabilitation experience required
• New graduates accepted with Level II clinical experience (Level I observation alone is insufficient)
• 3 years active Certificate of Clinical Specialty (CCS) credential preferred
