Physical Therapist Job in Grand Junction, CO

ADEX Healthcare Staffing is hiring a Physical Therapist for a home health position with SummitWest Care in Grand Junction, Colorado. This is a 91-day travel assignment starting September 8, 2026, offering a 5x8 day shift schedule at $2,482 per week.

About the Role

You will deliver in-home physical therapy services to patients following physician orders and agency protocols. Your responsibilities include conducting evaluations, Start of Care visits, reassessments, recertifications, and discharge visits. You will develop and implement individualized plans of care tailored to each patient's needs, provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention strategies, and educate patients and caregivers on treatment plans, home exercise programs, and safety strategies.

You will complete all required documentation accurately and timely in Homecare Homebase (HCHB), collaborate with agency clinical staff including RNs, PTAs, OTs, STs, and leadership, and participate in care coordination to meet patient needs and regulatory requirements. This role is independent, conducted in a home-based setting with one-on-one patient visits.

Requirements

• Current unrestricted Colorado Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver's license and reliable transportation
• Home health experience preferred
• Experience with geriatric and medically complex populations preferred
