Physical Therapist Job in Salida, CO

Heart of the Rockies Medical is seeking a Physical Therapist with Pelvic Health certification for an outpatient clinic position in Salida, Colorado. This is a 91-day travel assignment running five 8-hour day shifts per week, with a weekly pay of $2,741. The start date is November 16, 2026.

Role Responsibilities:

• Conduct comprehensive patient assessments to evaluate impairments, functional limitations, and disabilities
• Determine diagnosis, prognosis, and develop evidence-based intervention strategies
• Design and modify therapeutic treatment plans to address impairments and restore daily functioning
• Focus on injury and disability prevention through health promotion and wellness education across all age groups
• Maintain accurate, timely patient records including physician orders, diagnosis, treatment plans, progress reports, and discharge summaries
• Submit charges for services rendered promptly
• Engage in knowledge sharing with colleagues and community contributions to physical therapy research

Required Qualifications:

• Graduation from an AMA-APTA approved school of physical therapy
• Active Colorado Physical Therapist license or permit from licensing board while awaiting licensure examination
• Pelvic Health Certification required
• Minimum 4 years of experience in Pelvic Health
• Membership or eligibility for membership in the American Physical Therapy Association or American Registry of Physical Therapy
• Current CPR Certification
