<p><strong>Allied Position Job Notes:</strong></p> <p>Travel or Local Allied Healthcare Professional needed for ASAP start. A minimum of 2+ years of recent hospital bedside experience is required. Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Perform thorough assessments of patients&#39; physical abilities, movement limitations, and pain levels<br>• Develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals<br>• Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility<br>• Monitor patient progress, adjust therapy plans as needed, and document all care provided in accordance with state and federal guidelines<br>• Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use<br>• Ensure compliance with all regulatory requirements including Medicare/Medicaid documentation<br>• Participate in care planning meetings and interdisciplinary team conferences<br>• Supervise physical therapist assistants (PTAs) and aides as needed<br>• Graduate of an accredited Physical Therapy program required<br>• Current state license to practice as a Physical Therapist (PT) required<br>• CPR certification preferred<br>• Experience in SNF or geriatric rehab setting preferred<br>• May involve lifting, standing, walking, and assisting patients with transfers and mobility</p><p><strong>Facility:</strong></p><p>This skilled nursing facility provides comprehensive inpatient care, long-term care services, and short-term rehabilitation programs.
