Job Details:

• See patients across multiple care settings including acute care, outpatient, skilled nursing facility, and emergency department
• Plan, prepare, and conduct individually designed physical therapy treatment programs to maintain, improve, and restore physical functioning
• Perform and document initial patient examinations, evaluating data to identify problems and determine diagnoses prior to intervention
• Evaluate effects of interventions at various stages throughout treatment and adjust programs to achieve maximum patient benefit
• Administer manual exercises and therapeutic modalities to increase patient strength and reduce or prevent deformity and disability
• Oversee and direct patient care provided by physical therapy assistants to ensure quality and continuity of care
• Assist patients in reaching maximum performance and functional levels while helping them adapt to physical limitations
• Confer with patients, medical practitioners, and other appropriate personnel to plan, implement, and assess intervention program effectiveness
• Review physician referrals and patient medical records to determine appropriate diagnoses and necessary physical therapy treatments
• Assess patient pain interfering with optimal level of function and make appropriate physician contact for intervention
• Perform appropriate assessments and reassessments on all patient populations including pediatric, adolescent, geriatric, and general patients
• Formulate teaching plans based on identified learning needs and evaluate effectiveness of learning, including family as appropriate
• Coordinate and direct patient care to ensure patient needs are met
• Demonstrate knowledge and proficiency of all rehabilitation services modalities
• Maintain competency with required electronic medical record system for patient charting and billing
• Participate in performance improvement initiatives and Quality Improvement (QI) activities
• Current California Physical Therapist License required
• Current BLS certification required
• Previous experience in rehabilitative medicine preferred but not required
• Strong written and verbal communication skills required
• Basic computer knowledge required
• Knowledge of physical therapy machine operation, maintenance, and repairs required
• Ability to communicate effectively in English both verbally and in writing required
• Additional languages preferred
