Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Provide physical therapy services in home health setting<br>• Coverage areas include Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont)<br>• Additional travel outside assigned areas may be required<br>• Weekend coverage may be required<br>• Guarantee option: 36 hours guaranteed with minimum 27 points per week productivity requirement<br>• Non-guarantee option: payment made on per visit basis with no guaranteed hours<br>• Must be willing to see COVID+ and COVID PUI patients<br>• Various visit types including OASIS certifications, evaluations, routine visits, and discharges<br>• Visit points system: standard visits = 1 point (1.33 hours), OASIS Start of Care = 1.5 points (2 hours)<br>• Rate covers drive/travel time, visit time, documentation time, and phone calls to patients and doctors<br>• Required credentials: Current CA PT License, BLS, Driver&#39;s License, Auto Insurance, Auto Registration, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug Screen, Physical<br>• All requested time off must be disclosed at submission - no post-confirmation additions allowed</p><p><strong>Facility:</strong></p><p>This home health organization provides comprehensive physical therapy services throughout the Bay Area, focusing on delivering quality patient care in the comfort of patients&#39; homes.
