Physical Therapist Job in San Ramon, CA

You will provide physical therapy services in a home health setting serving the greater San Francisco Bay Area. Your coverage areas include Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont, with possible travel outside these assigned areas as needed.

Your Responsibilities

• Conduct OASIS certifications, evaluations, routine visits, and discharge services
• Manage visit points and documentation for each patient encounter
• Communicate with patients and physicians regarding care coordination
• Provide care to COVID-positive and COVID-suspected patients
• Cover weekend shifts as required

Compensation and Staffing Structure

You will earn $2,628 per week. Two staffing options are available: a guarantee option providing 36 hours per week with a minimum of 27 productivity points per week, or a non-guarantee option where payment is made per visit with no guaranteed hours. The rate covers drive and travel time, visit time, documentation time, and phone calls to patients and physicians.

Required Credentials

• Current California PT License
• BLS certification
• Valid Driver's License, auto insurance, and auto registration
• TB clearance (annual)
• MMR, Varicella, and HepB immunizations
• TDap (10 years)
• Flu vaccination (annual)
• Drug screening and physical examination

Assignment Details

This position begins May 4, 2026, and runs for 91 days on a day shift schedule. All requested time off must be disclosed at the time of submission; no additional time off requests will be accepted after confirmation.
