Case Management RN Job in Oakland, CA

You will serve as a Registered Nurse in Case Management at Sutter Health Alta Bates Summit Medical Center Hawthorne, where you coordinate care transitions and manage patient outcomes across the acute care continuum.

Schedule & Duration

• Five 8-hour shifts per week, days schedule
• Every other weekend required
• 91-day assignment beginning November 9, 2026
• Weekly pay: $3,080.16

Core Responsibilities

• Conduct patient initial and continued assessments, reviewing physician admission care plans and gathering medical, psychosocial, and financial information
• Formulate and implement transition plans in collaboration with patients, families, physicians, and healthcare teams
• Manage care coordination across discharge planning and inpatient acute settings, serving adult and geriatric populations
• Perform utilization management duties, reviewing medical records to confirm patients meet level of care requirements and monitoring length of stay
• Identify and address barriers to care progression, coordinating with multidisciplinary teams to resolve issues promptly
• Screen 30-day readmissions and develop effective transition strategies
• Identify patients appropriate for case management intervention through electronic health record review and staff collaboration
• Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient progression
• Communicate with financial counselors regarding uninsured and underinsured patients and coordinate appropriate referrals
• Maintain knowledge of CMS and Joint Commission Transitions of Care requirements and other regulatory standards

Patient Ratios

• 15 patients on days
• 25 patients on weekends

Required Qualifications

• Valid California RN License
• BLS certification (AHA or ARC)
• 5+ years of RN Case Management experience
• Discharge planning experience
• Inpatient acute case management (hospital) experience
• Proficiency with Epic charting system
