Case Management RN Job in Glendale, CA

Glendale Memorial Hospital and Health Center is seeking a Registered Nurse for a Case Management role in an acute care setting. This is an 8-hour day shift position with an every-other-weekend schedule and a 91-day assignment starting December 7, 2026.

Position Details:

• 5x8 day shifts with every-other-weekend (EOW) schedule required
• First-to-float availability when needed
• Weekly pay: $3,330
• Acute care environment focused on care coordination
• Electronic health record system: Cerner
• Hospital may cancel up to 3 shifts during this assignment

Your Background:

• California RN license required
• Solid experience in care coordination
• Prior experience with CA Dignity/CommonSpirit systems required for consideration
• Current ACLS and BLS (AHA) certifications required

In this role, you will manage patient care coordination within an acute care framework, utilizing your experience with Dignity/CommonSpirit platforms and the Cerner charting system. You'll be responsible for coordinating care activities while maintaining flexibility to support other areas of the hospital as needed. Your case management expertise will be essential in facilitating transitions of care and ensuring continuity for patients.
