Physical Therapist Job in Lakeport, CA

ADEX Healthcare Staffing is recruiting a Physical Therapist for a 91-day travel contract at Lakeport Post Acute in Lakeport, CA. This is a full-time position with an ASAP start date of October 21, 2025.

Position Details

• Schedule: 5 days per week, 8-hour shifts (days)
• Weekly pay: $2,920
• Setting: Skilled nursing and post-acute care facility

Your Responsibilities

• Evaluate and treat residents to assess functional status and rehabilitation potential
• Develop and implement individualized treatment plans aligned with resident goals
• Collaborate with interdisciplinary care teams to coordinate seamless patient care
• Document clinical findings and progress accurately and timely per facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives

Required Qualifications

• Active California Physical Therapist license
• Strong clinical assessment and communication skills
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented approach to patient care
