Physical Therapist Job in Wisconsin Rapids, WI

Aspirus at Home - Wisconsin Rapids is seeking a Physical Therapist for a home health role serving adult and geriatric populations across central and northern Wisconsin regions.

Role & Schedule

• Monday–Friday, 8-hour day shifts with variable scheduling
• Guaranteed 38 hours per week, with flexibility up to 40 hours
• Rotating Saturday coverage approximately every 6–8 weeks (compensatory day off provided that week)
• Low census call-back minimum: 2 hours
• No holiday rotation required

Clinical Responsibilities

• Evaluate, treat, and plan patient care according to Physical Therapy standards of practice
• Serve patients with orthopedic, neurological, and functional mobility diagnoses
• Maintain productivity target of approximately 4.5 visits per day
• Document in NetSmart EMR to meet regulatory and reimbursement requirements
• Use laptop for point-of-service documentation and cell phone for GPS navigation and communication

Logistics

• Occasional travel required to central and northern service regions
• Mileage reimbursed at government-approved rate
• Dress code: any scrub colors or business casual

Required Qualifications

• Bachelor's Degree in Physical Therapy (minimum); Master's Degree in Physical Therapy required
• Valid Wisconsin state Physical Therapy licensure
• Valid driver's license with proof of current car insurance
• AHA or ARC BLS certification
• NPI number
• Minimum experience in home health with adult and geriatric populations
• Demonstrated knowledge of Physical Therapy standards and practices
• Must reside outside a 50-mile radius of the primary location

Compensation & Duration

Weekly pay: $3,092. Assignment duration: 91 days. Start date: November 23, 2026.
