Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Provide physical therapy services in home health setting across assigned county territories<br>• Coverage areas include Area #8 (Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) and either Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) or Area #7 (Vallejo, Benicia, Napa, American Canyon, Yountville)<br>• Additional travel outside assigned area may be required<br>• Weekend coverage may be required<br>• For guaranteed positions: minimum 27 points per week with 36-hour guarantee<br>• For non-guaranteed positions: payment made on per-visit basis with no guaranteed hours<br>• Must be willing to see COVID+ and COVID PUI patients<br>• Visit types include OASIS ReCertification, OASIS RoC, routine visits, evaluations, discharges, and follow-ups<br>• Rate is all-inclusive covering drive/travel time, visit time, documentation time, and phone calls to patients and doctors<br>• Required credentials: current California PT license, BLS certification, valid driver&#39;s license, auto insurance, auto registration, TB test (annual), MMR, Varicella, Hepatitis B, TDap (10 years), flu vaccine (annual), drug screen, and physical exam<br>• Must comply with all requested time off disclosure requirements at submission</p><p><strong>Facility:</strong></p><p>This home health organization provides comprehensive physical therapy services throughout multiple counties in Northern California.
