Job Details:

• Home health setting with 6-8 skilled patient visits per day (minimum of 6 patients per day, may increase at manager's discretion)
• Self-scheduled visits and hours based on the needs of the home care office
• Required to have a valid driver's license and personal vehicle to conduct home visits
• 2 years of experience required
• Home health experience required; OASIS training preferred
• Cerner charting experience preferred
• Required certifications: BLS and valid state license (FL license or compact license required)
• Required skills: Activities of Daily Living, Orthopedics, Prosthetics/Orthotics, Home Exercise Program, Patient/Family Education, Safety and injury prevention, Evaluation Tools, and Billing/Documentation
• Patient population includes: Amputee, Arthritis, CVA Rehabilitation, Dementia/Alzheimers, Hand Injury, Traumatic Brain Injury, Lower Extremity, Multiple Sclerosis, Nerve Injury, Parkinson's Disease, Other Progressive Neurological Disorders, Spinal Cord Injury, Tendon Repair, and Upper Extremity
• Patient age groups: Adults and Geriatrics
• Equipment provided includes: Laptop, MiFi for internet access, and portable printers
• Charting systems used: BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
• Dress code: Any solid color scrubs
