PT - Physical Therapist

Job Details:

• Day shift hours from 10:00 AM to 6:00 PM
• 5 days per week, 8-hour shifts
• No weekend requirement
• No on-call requirement
• Dress code: Khaki pants and white polo
• Perform comprehensive diagnostic evaluations to determine physical and cognitive status of inpatients and outpatients
• Develop and implement individualized treatment plans with measurable goals, incorporating patient and family participation
• Provide physical therapy services including assessment, treatment plan development, implementation, follow-up, and discharge planning
• Maintain documentation including initial patient evaluations, daily and weekly progress notes, discharge summaries, and other appropriate notations
• Participate in patient care conferences, family conferences, in-service programs, and departmental meetings
• Counsel patients, families, and other involved individuals regarding patient evaluation, care, and treatment
• Initiate and complete discharge planning in a timely manner and provide effective patient education and training
• Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs as well as volunteer programs
• Assist with clinical aspects of the department including orientation of new associates and scheduling of patients
• Supervise Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers
• Participate in Performance Improvement activities and maintain departmental productivity standards
• Maintain awareness of community agencies for appropriate referrals and community resource networking
• Bachelor's, Master's, or entry-level Doctorate of Physical Therapy degree from an accredited college or university required
• Proficiency with all modalities and equipment used by a Physical Therapist required
• Current New Mexico Physical Therapist licensure required
• BLS certification from AHA required
