Physical Therapist Job in Albuquerque, NM

Advantage Home Care, Inc. is seeking a Physical Therapist to deliver skilled therapy services to patients throughout the Albuquerque area and surrounding communities. You will work independently in a home health setting, managing your own caseload within a 45-mile radius that includes Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen.

Your Responsibilities

• Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
• Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
• Deliver skilled interventions and maintenance therapy programs within your state-defined scope of practice
• Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
• Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress
• Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
• Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes
• Apply ICD-10 coding accurately for compliant documentation
• Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
• Contribute to Quality Assessment and Performance Improvement (QAPI) activities and periodic in-service programs

Required Qualifications

• Graduate of an accredited physical therapist curriculum
• Current Physical Therapist licensure in the state of New Mexico
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance for home visits
• Basic computer proficiency and willingness to learn EMR systems and OASIS documentation

Preferred Qualifications

• Two years of clinical experience
• Home health experience

• Duration: 91 days
• Weekly Pay: $2,384
