Case Management RN Job in Paramount, CA

Kindred Hospital Paramount is seeking an RN Case Manager for a 91-day travel assignment. You will work Monday through Friday, 8:00 AM to 4:30 PM, providing comprehensive case management services in an acute care setting.

Your Responsibilities

• Conduct comprehensive assessments and develop individualized care plans based on patient needs
• Coordinate care delivery and ensure optimal resource utilization across the care continuum
• Implement and evaluate care plans to promote quality outcomes and patient satisfaction
• Manage utilization review and discharge planning functions to ensure compliance with external review agencies
• Apply MCG guidelines to support clinical decision-making and appropriate level-of-care placement
• Serve as a patient advocate, addressing holistic needs including social, financial, and medical factors
• Collaborate with interdisciplinary teams to strengthen care coordination and continuity of care
• Support cost-effective care delivery while maintaining quality standards

Requirements

• Current California RN license
• Minimum 2 years of recent RN Case Management experience
• Experience in LTAC or STACH settings preferred
• BLS certification through American Heart Association required
