RN - Case Management

Job Details:

• Admit patients to hospice service and conduct comprehensive assessments of physical, psychosocial, environmental, safety, and developmental needs
• Develop individualized plans of care based on patient and family assessments
• Document observations and interventions related to the patient's environment
• Coordinate care with a multidisciplinary team
• Participate in the hospice program's quality assessment and performance improvement program
• Conduct follow-up visits as needed
• Cover territory including Oakland, Alameda, San Leandro, Ashland, Castro Valley, San Lorenzo, Hayward, Union City, Fremont, Newark, Milpitas, and neighboring cities as needed
• Minimum 1 year of hospice nursing experience required
