Physical Therapist Job in Lewistown, MO

Tide Health Group – Country Aire is seeking a Physical Therapist for a temporary, part-time position in long-term care. You will work 5×8 shifts during daytime hours with no on-call responsibilities and no required weekend shifts unless a holiday falls Monday through Friday.

Position Details:

• Duration: 91 days
• Start date: July 27, 2026
• Weekly pay: $2,534.40
• Location: Lewistown, MO

Your Responsibilities:

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
• Develop appropriate plans of care following regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
• Assess patient progress and adjust treatments accordingly in clinical records
• Evaluate treatment outcomes and modify services or make referrals as appropriate
• Provide consultation and counseling to patients, families, caregivers, and service providers
• Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries per regulatory requirements
• Document all required information in the electronic documentation system and maintain accurate patient records
• Record treatment minutes for all services provided
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and students as needed

Qualifications:

• Missouri state license required
• Minimum 1 year of physical therapy experience required
• Long-term care or rehabilitation experience required
• New graduates accepted provided they have Level II clinical experience (Level I observation experience alone does not qualify)
• Adherence to APTA and state-required practice acts, code of conduct, and ethics standards
