RN - Case Management

Job Details:

• Work Monday through Friday, 8:30 AM to 5:00 PM
• Coordinate management of care and ensure optimum utilization of resources, service delivery, and compliance with external review agencies
• Conduct comprehensive assessments, care planning, plan implementation, and overall evaluation of individual patient needs
• Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness
• Integrate functions of case management, utilization review and management, and discharge planning
• Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs
• Ensure strong interdisciplinary team (IDT) collaboration across acute care settings
• Apply MCG guidelines and discharge planning experience across acute care settings
• Minimum 2 years of recent RN Case Management experience required, with experience in a LTAC or STACH setting
• California RN license required
• BLS certification required (must be through the American Heart Association)
