Case Management RN Job in San Leandro, CA

Kindred Hospital - San Francisco Bay Area is seeking an RN Case Manager to join the team on a travel assignment. You will work Monday through Friday, 8:30 AM to 5:00 PM, for a 91-day contract beginning September 21, 2026.

Your Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans that address holistic patient needs
• Coordinate care management and ensure optimal resource utilization across acute care settings
• Implement discharge planning strategies using MCG guidelines to promote continuity of care and cost effectiveness
• Integrate case management, utilization review, and discharge planning functions to enhance patient satisfaction and quality outcomes
• Advocate for patients through resource allocation and collaborative problem-solving
• Serve as a key member of the interdisciplinary team (IDT), ensuring strong collaboration across departments
• Maintain compliance with external review agency requirements and regulatory standards

Required Qualifications:

• Current California RN license
• Minimum 2 years of recent RN Case Management experience
• Experience in a LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) setting
• BLS certification through the American Heart Association
