Med Surg RN Discharge Planner Job in Fort Defiance, AZ

Tsehootsooi Medical Center is seeking a Registered Nurse for a discharge planning role on the Medical-Surgical unit. This position offers 8-hour shifts Monday through Friday, 8:00 AM to 5:00 PM, with a 91-day assignment starting October 6, 2026.

In this role, you will:

• Perform case management and coordinate discharge planning care processes for age-appropriate inpatients with chronic diseases and special health care needs
• Manage patients across multiple units including Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit
• Maintain a tracking system for referred patients for admission with other healthcare facilities
• Perform chart reviews for requested services for return to facility or discharge home
• Attend staff meetings when scheduled

Requirements:

• Minimum 2 years of experience in discharge planning and Med-Surg
• RN experience in a clinical, medical/surgical, acute, and/or critical care setting
• Valid and unrestricted Registered Nurse license in any U.S. State or Territory
• Valid American Heart Association (AHA) Basic Life Support (BLS) certification required and must be maintained

Weekly pay is $2,721. This position is based at Tsehootsooi Medical Center in Fort Defiance, Arizona.
